A Practical Approach to Supporting Vulnerable Adults

The problems our community’s seniors and disabled adults face are complex and solutions do not always fall neatly within the lines of one agency. The County works to increase collaboration among the many agencies providing adult protective services through regular inter-agency meetings.
These meetings are held once a month at the Department of Social Services at 3433 South Higuera Street in San Luis Obispo and are attended by DSS staff and community partner representatives.
Staff from the County Probation Department, the County Office of the Public Guardian, local hospitals, the County Behavioral Health Department, and law enforcement agencies can request a meeting when they identify a senior or dependent adult or child who would benefit from coordinated services and the contributions of multiple partner agencies. For example, a homeless individual who has mental health and drug or alcohol problems who has frequent arrests and trips to the hospital might be recommended for coordinated services. When there is an individual identified in the community who is a frequent user of multiple public services, Adult Protective Services or a partner agency may want to request an Inter-agency meeting to develop a coordinated services plan.

Prior to the meeting, a signed release of information is required that enables the client to identify with which agencies his/her personal information can be shared.
